Form Inquiries Not Going Through/ No Confirmation

I recently made a new 'contact us' form page, made a block and placed it on my contact page. The form looks the way I want, but when I sent a test inquiry and clicked 'submit', the page just reset. I did not receive a test email nor did I see any confirmation that the inquiry went through. Reasons why this might be happening?

I am having the same issues 4.11.2 Ultimate. Webforms not working . Customers not getting order confirmation despite notify customer checked.

It was working well until a week ago. Not sure whats gone wrong.
