Declined orders create subscription plans

HELP!!! I have products tied to recurring plans. When an order is declined (bad credit card) it goes ahead and creates a subscription. If the customer attempts to pay again and is declined again, it creates another subscription plan for the original order number. Everytime the order is processed and is declined it creates a new subscription plan. Seems like subscription plans would only be set up for orders that are actually successfully processed with payment. Is this a setup issue?
