With the cloud version of ultimate now being significantly less than the annual customer care cost and including customer care and hosting to boot I tossing up wether I bother continuing with my old lifetime ulimate license. The later might be ok for someone who doesn’t need support services but I find myself calling on them regularly, does anyone see a downside to the cloud service? i.e. is there anything (besides custom themes and add-ons) that you can do in the on premises version you can’t do in the cloud version?

I mean, being unable to install any third-party add-ons is a huge difference. You can’t add any new payment providers, shipping provider, QOL changes and so on.
I find No-Code being more like “extended trial version” of CS-Cart rather than a complete e-commerce solution, exactly for this reason.
The main purpose of Multi-Vendor No-Code is to get up and running quickly with minimal effort and expense. It is the easiest way to test a theory, build an MVP, and if it is successful, you can move your project to the On-Premises version and grow your business as you wish, without the limitations of No-Code.
So the No-Code is more about “Launch Fast” and the On-Premises is more about the part “Scale Vast”
