Changing Blocks/editing Layout Only For Mobile Devices

Hello. I want to change blocks/grids layout only for mobile devices. For example, I want My Account block in “Header” grid for standart windows browsers, but when someone visits by using mobile devices the My Account block has to be in “Top Panel” grid.



How can I do this?
