I would suggest that you don't follow the instructions in the readme.txt and instead just take what you want from the included stylesheet. You also are going to have to snag the images from the source that are defined in this stylesheet, instead of calling them from their domain: http://csscheckbox.com/checkboxes/lite-green-check.png. I wouldn't want to rely on csscheckbox.com to provide me with these graphics. Snag them from their domain and upload them to your own server - change the path to your own directory.
Good luck, it looks like a fun CSS project to tinker with. I think I might try to implement a couple these myself.
