I have one doubt in custom addon because i need to create one new page in cs-cart through custom addon. the page is appear but i unable to extend header and footer in tpl file and here i am shared my new page .

I need to create one custom addon in cs-cart , that addon create one new page (which means seperate page ) in cs-cart because it has some new elements but my doubt is how to include header and footer in my new page because my page look like below

On design -> layouts create new layout page for your custom page, then enable unique header and footer there and disable necessary blocks
