- sending out a promo code for customers to use.
- don't want them to be able to use it for items in category "sale"
- i can exclude items in category "sale" but if a customer orders a second item, that's not in "sale" category, the "order discount" i've set doesn't apply to the second item either.
- how to i make a coupon code that works even if a customer has one item in their order from a category in which the promo is restricted?

it sounds like you’ve a fault in your discount rules.

I have some of the same problems. I want to give customers a discount on most items except a few specific categories (or product that are on sale already). I am struggeling to get the setup right.

I do not understand your specification from the first point. If customer has in cart one product from Sale category, and one product from other category, promotion should be activated or not activated?
I think you may be interested in one of our add-ons, which allows you to discount only products from certain categories:

Just create catalog promotion and exclude necessary categories from it
