When creating a promotion select the Once per customer condition.

I would recommend NOT using coupon/customer but instead having a single coupon related to your promotion. The cart itself will handle the "once per customer" limitataion. Otherwise, it becomes a performance issue when you have tons of promotions that are active but not valid for the order at hand. At every add to cart, the promotions are scanned to make adjustments to either the item price, shipping or order discounts.
