Most likely reason the following urls are being denied as it references the mainpage (index.php) and the string isn't considered to be anotherpage via the webmaster tools

As Jesse mentioned, you can use robots.txt to not allow future indexing, but that won't help you with getting the pages out of Google. I've used their request tool to have pages dropped and it always worked. Are they stating why it's not letting you?
I know one of the options is to request removal of pages that don't exist anymore...not sure if you want to go this far into it, but you could change file names to impact the URLs and then request them to be dropped from Google because they no longer exist.

I will try it out on my test store and see if it removes index.html from the index
If you're running the SEO module just add the "index.php?" string which will save you adding the rest

To prevent all robots from indexing a page on your site, place the following meta tag into the <HEAD> section of your page:
<META NAME="ROBOTS" CONTENT="NOINDEX, NOFOLLOW">

On a side note, what exactly do we want indexed or not?

What is cpanel 11?If you are using cPanel 11 there is an "Index Manager" that will allow you to choose what or what not you want indexed.
On a side note, what exactly do we want indexed or not?
You don't want Cr@p pages in the index it will not allow genuine pages to get a high pr / ranking
like all I want indexed in my main page and products
who wants their password recovery to be indexed???
I believe shoemoney(blogger) added a lot of things to his robots.txt and his lower pages serps went up, he wrote about it in his blog
and it kind of makes sense, if you have 400 pages sharing pr/strength, you make it 100, the 100 pages are going to get stronger

Again.. robots.txt does it all for you.
Cpanel is a linux hosting "platform" for clients which provides a simple GUI for managing your website(s).
If you don't already have this your missing out on life pleasures.
