File Upload Field Addon?

Does anyone know of an addon that allows for file uploads in say the checkout form?



Here's the scenario we're needing to find an answer to. We have a client with an eye glasses store and upon checkout, customers will need to either type in their prescription information or attach a PDF or JPG of their prescription or both…



Can anyone think of an addon or a way to accomplish this?

You can use the default type of product options - File. So if a customer orders glasses for different people, he will be able to upload different prescription info for each product. Will it be suitable for you?

Think you can do this already. Was able to do it in 3.x anyways.

For example, you have product “Glasses A”


  • Log in the admin panel and go to the product details page
  • Go to the Options tab and click on the Add option button
  • Specify the option name (e.g. Upload prescription data), select the File type and click on the Create button
  • Go to the store-front on the product details page and check the result

You could set them all up on a product by product basis with global options using dropdown as selector.

set up a global option called axis and have all the axis values in that, same for cyl, same for sphere etc.

Then customer can add them at the product page. you would only have to set these up once than add them all from global options to each product

@johnbol1, Is there a way to control the layout of of the options so that we can emulate this screen from Zinni Eyewear?
