I will share me first Free Addon with you.
This feedback form will allow customer send feedback about your site.
is simple and high racomended to stay connectet with your customers.
the script send a email with nessasary entries.
Feel free and give some feedback how you customer use it .
the smiley select not works in IE8 and 7 = Maybee someoune will give a look in the javascript.
If you like to edit the language, simple edit in Language section from Cs- cart.
Feedback button include in PSD to customize your language.
EDIT: add send_feedback_reverse.zip with smilie position change. *change the language for smilie title

You might change the text a little as it has some errors in it. And, I would begin with poor and end with excellent.

jes i’m not really goot with english 
normali European people → looks and read from left to right
click on the left side first beqause is the first thing where if see.
For this i add all positiv suggetion are on the left side first.
Hello, for what version it is? I have Cs-Cart ultimate 3.0.3. and is not working for me!

Look here: http://zspotmedia.ro/ (in the left) is just a custom form with name/email… etc… I think it is a good way of putting contact form on a webpage!
my feedback addon is a servey about your site. 
anyway is not planed a slideout form like this or a contact form.
maybee if i have time.
but this i share beqause i need it on my site.
It is a suggestion, I don't think slide or pop-up will matter, but just a form (we can build it from tpl) will be great!

popup works only if customer click on the feedback badge on theleft side. see picture 2.

Yes emails will send to Help/Support department e-mail address
