or do people not deal with spam much at registration?
There's no addon because it's already integrated into the cart. Settings > Image Verification.
Image verification but not Q & A. Image Verification get busted by screen readers eventually. Thats why captcha has gone done the pooper in years

Thats reassuring to know.
If you dont mind me asking how large is your daily user base? Just a little weary it being a commerce platform and the security seems a little behind.
We get 80 - 150 users a day and have no problem with spam. 2 or 3 times a year we may get a spam message sent to us. As long as we do not reply they do not have an actual email address now to spam to.
We have had the store up for a few years now.

You should also modify the existing Image Verification strings and grid based upon your requirements.
A smaller grid with strictly numbers is easy to read but difficult enough for a screen reader to fail
