When are reward points not accumulated?

I have a customer asking why she didn't receive rewards points from her last order & I don't know how to answer her question. In looking at her reward points log I am confused - all it shows is:



-75 points - order placed

+75 points - Open —› Paid (The points used in the order were added to user account.)

-75 points - Paid —› Complete (The points used in the order were subtracted from user account.)



And on this particular order is shows 75 points in use.



Point balance 0.
