Currently I'm on version 2.0.1. My designer designed my website by modifying the default blue skin. It wasn't done cleanly so I'm afraid of the design completely messing up if I decide to upgrade as I'm sure many things have changed since that version.
Is there a way I can test the upgrade to the current version and see what happens, but at the same time have the option of completely going back to what I have currently if it looks bad? I do plan on redesigning the look eventually, but probably won't be for 4-6 months. There are some features that have been included in the updates that I want to use.

There is the ability to revert any upgrade after it is installed. To do it, you should go to “Administration->Upgrade center-> Installed updates” and click on the “Revert” link next to the necessary upgrade.
Also, please note that you can resolve the conflicts caused by the code changes, following the instructions here:

Besides, you can choose some of the standard skins after the upgrade and use it until you prepare the necessary design changes.
If you are still on 2.0.1 there are a ton of upgrades between that version and the current version. I know the revert thing is good, but I would question if it is that good.
Your best bet would be to set up a copy of your site on a test server and then go through the upgrade process.
Also, be sure you back up everything.
One thing for sure is that the way a lot of the links were done has seriously changed between your version and the current one. So not only will your design be jacked up, but there is a good chance that your links will also be messed up.

If I were to set it up on a test server, would I be able to upgrade since I only have one license?
If you're really going from 2.0.1 (fist) to 2.2.2 (current) I would export all products, customers, orders and anything else I could and then I'd throw the 2.0.1 installation away and do a fresh install. Then import everything you exported.
There were so many upgrade issues in the earlier releases of 2.0 that the odds of you actually getting through the nearly 20 upgrades you will have to do are slim to say the least.

Yeah, that would be a painful experience to say the least!
And after all of that effort & frustration you will still most likely end up with a train wreck of a site & wish you would have just manually migrated your DB & recreated your skin design…
