Shipping Rate Error

I have shipping set up for flat rates depending on Price.

All works well until you add more than one of the same item to the cart. Doesn’t seem to matter which item you try it with. Shipping rates are all off unless there is only one of any item.

Everything seems to be set up correctly. Am I missing something?
