Anyone know how to configure the cs cart so that you can use both the per item shipping and the over a certain amount is free shipping?

You can use promotion to do free shipping offer.

Admin/catalog/promotions



add cart promotion

give it a name, then in conditions select add condition, if order sub total or value etc is over XXX amount,



then go into bonuses and click add bonus, select free delivery and your done I think
