FYI:- Pop up fields have alignment problem and fields are not validated properly.

It depends on your design. It's just a form from well-known “Form builder” add-on, so it means that Form builder add-on has alignment problems with the woody default theme.

We decided to use simple input fields for demo purpose but you can use the “phone”, “e-mail” and others special elements with the built-in validators.
