Form in Block Template

Hi All



I have looked around the forum and kb and not found a thing about this topic. Perhaps I’m looking in under the wrong topics, so any help if appreciated.



I want to create a form within a block. The reason is so I can include this form on the homepage as well as within the product tabs.



What is the best way to achieve this?

Any solution to this as of yet? I too would like to add a quick contact form on my pages sidebar
