I want to be able to have an additional form within the checkout form that allows a customer to enter 1-10 additional students/users (name, phone and email) that will be registered for a product. I would like the user to select the number of students from a drop down list and then display that number of input rows.

In addition, once the user has selected the number of students, I would like to multiply the flat dollar shipping fee that we are applying by the number of students.

Any guidance on the best way to approach this. I realize that there will be some custom coding for this so I am really looking for an approach.

I have changed this request to be a bit simpler.

I want to be able to have an additional form within the checkout process that allows a customer to enter 1-10 additional mail to users (name, phone and email) that will be associated to a product. Like multiple shipping addresses.

You can find a lot of developers on this forum. Hire someone to create such modification for you
