we have add some new option in apply to vendor form file is :

/home/bigstree/public_html/design/themes/energot/templates/views/companies/apply_for_vendor.tlp

here new option we added :

wk2vrm.jpg

now how can we add this detail to our data base and admin panel also ??

please provide information in detail
